How to Apply for MATCH Chicago

Applications from Chicago-area schools can be submitted starting February 2, 2015 and must be received by November 2, 2015.
All MATCH application information is kept strictly confidential.
Schools
- Application sections should be downloaded, printed, and signed.
- School professionals are responsible for completing Section 1 and Section 2 of the application.
- School professionals are responsible for making sure that donors complete Section 3 and Section 4 (Donor) or Section 4 (Foundation) of the application.
- School professionals should submit all four sections of the application as one scanned pdf file in one email message to info@dayschoolmatch.org by Monday, November 2, 2015. Incomplete applications will not be reviewed. Please double check that your scan is legible before submitting.
- Applications may also be submited in hard copy and mailed to MATCH, The AVI CHAI Foundation, 1015 Park Avenue, New York, NY 10028.
- Please also submit Annual Reports from the past three years if available.
- The school and donor will both receive an email from MATCH confirming that all application components were succesfully received, and the application is under review.
- MATCH will contact the day school professional directly if additional information is required.
Donors
- Application sections should be downloaded, printed, and signed.
- Donors are responsible for completing Section 3 and Section 4 (Donor) or Section 4 (Foundation) of the application.
- Donors should submit completed Sections 3 and 4 to the day school professional.
- The school and donor will both receive an email from MATCH confirming that all application components were succesfully received, and the application is under review.
- MATCH will contact the donor directly if additional information is required.
